Dear Sir or Madam,

I recently received a copy of my credit report and need to dispute inaccurate information that is in my file. For your convenience, I have enclosed a copy of the report and encircled the information that I believe to be incorrect.

(In the body of the letter, you should describe the inaccurate information in detail-include the name of the creditor and identify the type of item that it is. You will also want to explain why you believe this item is inaccurate.)

I am requesting that the above mentioned item be removed from my report to correct the error. Prompt attention to this matter is appreciated.

To assist you in your investigation, I have enclosed several documents that support my position. (Only use this last sentence if you have support documentation to enclose, such as payment records or court documentation.)

Sincerely,

Your Name (Print and Sign)
